Learn More About Dementia

Many of us, especially seniors, have heard or known someone with Alzheimer’s disease and have an idea of the devastation it causes. While there are other types of diseases categorized as dementia, Alzheimer’s is by far the most common form of dementia accounting for 60 to 80 percent of cases. You should know dementia is not actually a specific disease but refers to a decline in a person’s mental ability to such a degree it interferes with their daily life.    

It is also important to understand dementia is not a normal condition of aging but rather caused by damage to brain cells that affect a person’s memory, reasoning and other cognitive skills including an ability to communicate. There are several types of clinical dementia including: Alzheimer’s disease; vascular dementia; dementia with Lewy Body; and frontotemporal dementia. Mixed dementia is the term used when a person has both Alzheimer’s and another form of dementia, which is causing their mental decline. 

The Alzheimer’s Association says the “most common early symptom of Alzheimer’s is trouble remembering new information because the disease typically impacts the part of the brain associated with learning first.” As the disease progresses, the person is robbed of many other abilities such as speaking, swallowing and walking. 

To help get the proper diagnosis and rule out other causes that may be treatable such as side effects from a medication, it is important to see your doctor. If warranted, they may have you see a specialist that can do further testing.

How Technology Can Help Seniors

The US Census Bureau shows by 2030 as many as one in five Americans will be 65 years or older, and by the mid-30s, there will be slightly more people over 65 than people 18 years or younger. Our increasingly older population is due to better health care enabling people to live longer. Scientific advancements should also help people maintain better cognitive health.

The large population of seniors has led to tech companies addressing their various needs with new devices designed to make them more independent and live fuller lives. Problems like medication management are being addressed by smart medication management systems that can handle up to as many as ten different medications. Reminders are connected to WI-FI and can even go to family members if the person misses a dose.

COVID-19 taught us the advantages of Telemedicine and is ideal even now for those who may have mobility issues or no longer drive. Video systems like Zoom, Skype or Facebook allow seniors to stay connected to loved ones face-to-face. Stretching Exercises are Important as You Age

Health professionals say it is important to incorporate stretching exercises into your routine, especially the older we get as our flexibility naturally decreases. As joints become stiff, and tendons and ligaments tighten up, we become more susceptible to injuries as well as chronic pain and poor posture. In fact, studies show men’s flexibility drops sharply at around 70 years of age. 

Stretching exercises are easy to learn, relatively easy to do, and should not take more than five to ten minutes. Urvashi Chogle, a physical therapist at Harvard-affiliated Spaulding Rehabilitation Network, says the shoulders, hips, knees, and spine lose the most flexibility.  He explains, "This makes many everyday movements more difficult, such as reaching overhead, squatting, twisting, and bending over." 

Of course, before you begin any new stretching exercises, you need to consult with your healthcare provider. He or she may have suggestions on how to safely get started or direct you to your local community senior center or other organizations for classes. Additionally, the Internet has a lot of information on how seniors can improve their flexibility with stretching exercises done in a chair, standing up, or if you can, on the floor. Understanding How Cognitive Reserve Protects Brain Health

The Harvard Medical School says to understand cognitive health it is important to recognize the concept of cognitive reserve, “your brain’s ability to improvise and find alternate ways of getting a job done.” Experts say a good way to think of it is like a powerful car with another gear that suddenly accelerates to avoid an obstacle. Thus our reserve can help the brain change the way it operates and make added resources to cope with the challenge.  

So how do we build cognitive reserves? Research shows it is developed through a lifetime of education and curiosity, which aids the brain in coping with the failures or declines it may face. The National Institute on Aging says cognitive reserve could account for the individual differences in susceptibility to age-related brain changes or Alzheimer's disease-related pathology. Evidence shows some people can tolerate more of these changes than others and still maintain function.  This is proven by studies in which people with higher education and higher occupational attainment appear to have a reduced risk for Alzheimer’s.  

While much of our cognitive reserve is developed over a lifetime, Harvard Medical School doctors say it is important to continue to engage our brain as we age. Examples of good brain activities to increase our reserve include learning to play a musical instrument or to speak a new language, doing crossword puzzles and engaging in high levels of social interaction. Regular exercise can also improve cognitive reserve.  

Other ways to protect our cognitive health and its reserve is to take care of our physical health especially when it comes to managing high blood pressure, diabetes and heart disease. A healthy diet and managing stress is also important.

Pets are Beneficial to Seniors

Research has shown pets are beneficial to seniors. A Place for Mom’s website reports on one study, which found “15 minutes spent bonding with an animal sets off a chemical chain reaction in the brain, lowering levels of the fight-or-flight hormone cortisol and increasing production of the feel-good hormone serotonin. The result: heart rate, blood pressure and stress levels immediately drop.” Researchers explain interactions between human and pets can lower cholesterol levels, fight depression and may even help protect against heart disease and stroke. 

Pets can teach seniors to live in the here and now as they do, and dogs and cats are great at offering their owners unconditional love, emotional support and affection. In fact, animals uplift the spirits of those in hospitals, convalescent homes, mental health centers, nursing homes, and assisted living communities through pet therapy programs. Moreover, pet therapy has proven beneficial for people with Alzheimer’s disease and other forms of dementia. Experts point to the animal’s full acceptance and non-verbal communication as being soothing for patients.

Gum Disease May Increase the Risk of Alzheimer’s Disease

Researchers at Tuft University recently reported a link between gum disease, also known as periodontitis and periodontal disease, to an increased risk for Alzheimer’s disease. Periodontitis is a bacterial infection within the soft tissue of the mouth, and “about 47.2% of U.S. adults over 30 and 70% over 65 have gum disease,” according to the Centers for Disease Control and Prevention (CDC). 

The study, which was done with mice, discovered the correlation, but researchers hope the findings can be used to someday help reduce cases of Alzheimer’s disease. The authors cite Fusobacterium nucleatum (F. nucleatum), a bacterium that causes gum disease and is already linked to kidney disease, diabetes and coronary problems to be the problem. 

The lead author, Dr. Jake Jinkun Chen, professor of periodontology and director of the Division of Oral Biology at Tufts University School of Dental Medicine explains the purpose of the study is to identify and characterize the association of induced periodontal disease with Alzheimer’s disease. He says as F. nucleatum generates “inflammation both locally and systemically, it affects the Alzheimer’s disease phenotype, as Alzheimer’s itself is an inflammatory disease.” Additionally since the bacteria migrate and penetrate the brain, it can worsen the symptoms and signs of Alzheimer’s disease. 

Researchers plan further studies in humans and say although there is not a proven link yet; good oral health benefits our overall health. They also say to decrease our risk for Alzheimer’s, it is important to maintain a healthy diet and exercise.

Talking to Children about Alzheimer’s Disease

When a grandparent or another close family member or friend suffers from Alzheimer’s disease, it can be difficult for children to understand especially if they are very close to the person.  Most likely they have questions and concerns regarding the disease and may be struggling with a range of emotions including sadness and confusion. They may not know how to act around the person anymore or be frightened by the “strange behavior.”

Having grandma or grandpa call you by mom or dad’s name or asking the same question again and again are just some of the strange things kids see happening to a beloved grandparent. They may worry Alzheimer’s is contagious and it is important they know it isn’t but rather a degenerative disease that happens in the brain. If the person lives with them, teenagers may be embarrassed to have friends around due to the person’s behavior, and sometimes children withdraw from the person or lose patience.  

The Alzheimer’s Association suggests sitting down with your child and telling them while the person may forget your name, they can still feel your love. Encourage questions and provide honest answers, and perhaps together you can learn more about the disease. The experts offer ideas on things kids can do with the person such as listening to music, setting the table, working on a puzzle, reading a book or looking at family photos. As the person’s disease progresses, make sure your kids feel comfortable asking more questions. The Future of Osteoarthritis Treatment

Researchers say a future of hip or knee “replacements” done without surgery may be on the horizon. By employing a combination of drugs and stem-cell therapies, it may be possible to “repair cartilage before osteoarthritis sets in or an operation is needed.”  

Osteoarthritis, commonly afflicting the elderly, is a degenerative disease attributed to "wear and tear" but can also be caused by earlier sports injuries, obesity and other trauma to the cartilage around the knee, hip or shoulder joints. Experts explain the loss of cartilage, which does not have its own blood supply or nerves, limits its ability to mend itself and is the major cause of joint pain and arthritis affecting over 32.5 million people in the U.S. In 2017 alone, there were 754,000 knee replacements done and 448,000 hip replacements.  

Michael Longaker, a professor of surgery at Stanford University School of Medicine and co-director of its Institute for Stem Cell Biology and Regenerative Medicine, explains the goal of his research is to not wait for osteoarthritis to develop, but to halt the degenerative process so people never need to have a joint replaced. His team’s focus is on programming stem cells to become cartilage cells. Other researchers are working to develop drugs that stop the progression of “wear and tear” on joints. Researchers are also looking at methods to “effectively deliver new cells and compounds” as well as designing materials to help new cells integrate with existing tissue. Scientists admit they still have a ways to go before they will have alternative therapies in place but remain optimistic.

A Poor Diet May Increase a Person’s Risk for Alzheimer’s Disease

Researchers have found evidence suggesting genetics, environment, and lifestyle can affect our risk of developing Alzheimer’s disease and dementia. A poor diet is one of those lifestyle choices that causes high blood pressure and heart disease and increases a person’s risk for Alzheimer’s disease. 

WebMD.com reports the MIND diet; a combination of the Mediterranean and DASH diets has shown promise in reducing the risk or slowing cognitive decline. They cite early studies showing MIND lowered the “risk of Alzheimer's by 53% in those who follow it closely and by 35% in those who follow more loosely.” In 2018, the MIND diet recommendation was expanded to include older adults who have suffered a stroke. 

To adhere to the diet, weekly meals should include food from the following groups: green leafy vegetables, other vegetables, nuts, berries, beans, whole grains, fish, poultry, olive oil, and wine. They say a good way to start is with a daily salad, one other vegetable, and three servings of whole grains every day. In terms of vegetables, collard greens, kale, and spinach have been found to be especially healthy. 

Poultry should be eaten once a week, fish twice a week and beans every other day. Nuts and berries make good snacks, blueberries and strawberries being some of the best. Olive oil not only adds a great taste to foods but has been shown to improve brain function over the long term and protect against dementia.
